Inbreeding increases the risk of deformity and disease, but it doesn't guarantee it. An inbred puppy can be perfectly healthy. Inbreeding to make certain traits breed true has been done for centuries. But it is very carefully controlled. Breeds weren't created by inbreeding the same line of dogs over and over, new blood was added and the inbred dogs were often generations apart or not immediate relatives.
