If you had stopped when it asked for the PUK code it would have been fixable with a simple phone call to customer service (or if you happen to have AT&T you could have gone to their website) but since you kept messing with it the SIM card is now "fried" and there is no reviving it. The ONLY choice now is to go to a store and BUY a new SIM card. They cost around $25-$30
